TCG System Access Request

Requesting User: Download the PDF form by clicking the 'Download Form' button below. Fill out the Top section and the User Information section.

If you are an In-Country User: Send form to the SCR/FLO (or Program Manager if no FLO is assigned) for their signature.
If you are an SCR/FLO USER or USG Program Team User: Send form to the Program Manager for their signature.
If you are a CONTRACTOR (i.e. LM AERO PSE): Send form to your Supervisor for their signature.

FLO / SCR: If this is an IN COUNTRY USER AND there is an in-country security manager, they fill out and sign the FLO / SCR section, otherwise the SCR/FLO will fill out this block. After completing this section, send form to the Program Manager for their signature.

Program Manager/Supervisor: Fill out the Supervisor / Program Manager section then send form to TCG for final approval.

NOTES:

Make sure all signature blocks are digitally signed. IF unable to digitally sign, ensure signature is LEGIBLE
Program Managers - ensure you have the TCG Chief's approval for contractor access PRIOR to routing a contractor employee's access form to gain access to the database.
