- If you are an In-Country User: Send form to the Database Manager for final approval.
- If you are a CONTRACTOR: Send form to your Country Representative for their signature, then to the Database Manager for final approval.
NOTES:
- Make sure all signature blocks are digitally signed. IF unable to digitally sign, ensure signature is LEGIBLE
- The TCG Chief's approval is required for contractor access PRIOR to routing a contractor employee's access form to gain access to the database.
